Job Title: Community Health and Wellbeing Coach
Hours: 35 hours per week Monday - Friday plus 1 in 4 Saturday mornings (job share considered)
Salary: £25k - 20k per annum (depending on experience)
Contract: Until March 2024
Accountable to: Chief Officer
Location: South Tyneside
We are currently recruiting for Age Concern Tyneside South (ACTS), a registered charity which promotes the wellbeing of older people living in South Tyneside, improving their quality of life and helping them maintain their independence.
This service will enable older people with mental health issues to actively participate to making positive changes and reduce the risk of people not being able to access the services they need.
We have an exciting opportunity for a Health and Wellbeing Coach to join our team. This post will work within ACTS alongside the South Primary Care Network (PCN) team, managing a case load of referrals, delivering an excellent service focussed on the needs of the individual.
You will work within the community in partnership with the South PCN team, working with people referred to the service and provide appropriate support based on the needs of the individual. You will coach, motivate, and set goals with clients to help them achieve personalised health and wellbeing care plan objectives.
You must have NVQ (or equivalent), or a relevant first level professional qualification or working towards this, and training in motivational coaching, behavioural change and goal setting. You will also have experience of working in Health and Social Care and delivering motivational interviewing and behaviour change.
